This is an example of using a graphical skin, which can often be more attractive for a stand-alone installation of the script or where it might be installed in a subdirectory and just linked to from an existing site.
Optionally, you can simply define an HTML header and footer in Admin/Appearance section, which is a quick way to create a form for integration into an existing website or use as a stand-alone upload page.
